The Aging Network can play a key role in offering evidence based health promotion programs as an entry point into the health care arena. Participants learned strategies for reaching out to health care providers and how to demonstrate value to establish contracts and successful partnerships for reimbursement. The Healthy Living Center for Excellence has contracted with a statewide health plan for a variety of evidence based programs including outreach and reimbursement for members attending programs. This session highlighted some of the key elements to securing contracts and critical strategies for maintaining them.
