They're just not that into you.

I am not sure the point is do people care or not. It is how to retain the right contacts, and filter the other ones. If you want to work with loyal people, with people you can trust, then return phone calls, and forget those who do not. As strange as it can sound: it is time saving! And in crisis situations it allows to react very fast (better have one friend who answers, than 20 who might help).

How can you figure out the right people to respond to and who not to? is that short term thinking?

I thought the "who to answer to" point, was addressed in the video... The "who not to answer to" is also addressed at the end of the video.
Returning phone calls can be seen as a self discipline having in mind the long term. You want to belong to the small group of reliable people, not to the majority who does not care…
Each and every time you take the decision to return or not a phone call from someone you know, you are stepping toward one of the two groups.
Each and every time you are returning the phone call of someone who consistently hasn't returned yours in the past, you are loosing time you could spend with "returning phone calls people "…

No doubt it’s something that bothers many people... who likes to feel dissed!
But, as you stated, people don’t return calls because (with rare exception) they don’t feel the need. Or, like recent movie title says, “they’re just not that into you.”
It happens rarely for me now. But, if it does, I’ve learned to accept it and, if anything, use it as motivation to do a better job of elevating my brand and professional image.
I love your honesty and sincerity, Barry. I also agree with, and relate to, much of what you say. But as you know, image is everything. And my concern about the video is that rather than reinforcing your image as “successful, sought-after expert” it conveys an image of a frustrated solopreneur, working at home, who doesn’t get calls returned.
